General comments
In verband met het voorkomen van azole resistentie wordt met dubbeltherapie gestart en dient men alle moeite te doen de verwekker aan te tonen.
Na gevoeligheidsbepaling kan de therapie aangepast worden. In geval van azole resistentie kan met ambisome al dan niet met een echinocandine doorbehandeld worden; niet met echinocandine monotherapie.
In geval van Mucormycose wordt behandeld met ambisome 5mg/kg, maar staat chirurgische interventie in het algemeen op de voorgrond.
Definities van invasieve pulmonale aspergillose bij oncologie patiënten:
Bewezen (proven):
- histopathologie (hyphae) vanuit naald aspiratie of biopt plus weefsel beschadiging
- EN positieve kweek van longweefsel OF positief aspergillus antigeen in serum of BAL
Waarschijnlijk (probable):
- neutropenie >10 dagen of GvHD behandeld met systemische immunosuppressieve therapie of > 3 weken corticosteroid gebruik in de voorgaande 60 dagen
- plus microbiologisch criterium: kweek van sputum of BAL vloeistof; of pos. directe kleuring voor schimmel in sputum of BAL vloeistof; of pos. antigeen test in BAL vloeistof of serum (OD index ≥ 0.5)
- plus specifieke CT afwijkingen: goed-afgelijnde nodules, nodules met halo-teken of crescent-teken of holtevorming binnen gebied met consolidatie
Mogelijk (possible):
- neutropenie > 10 dagen of GvHD behandeld met systemische immunosuppressieve therapie of > 3 weken corticosteroid gebruik in de voorgaande 60 dagen
- plus specifieke CT afwijkingen (zie bovenstaande) bij neutropenie zonder een microbiologisch criterium (zie microbiologische criteria bij 'probable')
Onbekende betekenis:
- neutropenie > 10 dagen of GvHD behandeld met systemische immunosuppressieve therapie of > 3 weken corticosteroid gebruik in de voorgaande 60 dagen
- plus aspecifieke CT afwijkingen of geisoleerd microbiologisch criterium (zie microbiologische criteria bij 'probable'
- of 2 minor klinische symptomen: long symptomen (hoest, thoracale pijn, haemoptoë, dyspnoe); pleurawrijven; elk nieuw infiltraat; pleuravocht
